Scheduling COVID-19 Vaccinations

Summit County Public Health has vaccination appointments available for Thursday, April 22, and Friday April 23. To book your appointment directly click here.

Note that appointments are available on a first-come, first-served basis. If you are unable to schedule an appointment for this week, follow the instructions below to have your name added to our list through PrepMod and we will notify you of upcoming vaccination appointments.

Add Your Name to Our List for Vaccine Appointments with PrepMod

Summit County is now using the PrepMod pre-registration system for COVID-19 vaccination appointment requests. The online registration system allows individuals to put their name on a contact list to request vaccination appointments in Summit County, as well as other counties.

Public Health officials will use a lottery process to randomly select individuals when appointment are available.

Summit County encourages ALL individuals to register to receive the vaccine. All individuals age 16 and older are eligible.

Community members are encouraged to register with PrepMod for appointments with SCPH as well as register directly with partner providers to increase their opportunity for receiving a vaccine appointment as quickly as possible. Partner providers are listed below the PrepMod instructions on this page.

COVID Vaccines – Regarding Different Manufacturers and Timing Between Doses

  • Currently, the U.S. Food and Drug Administration (FDA) has authorized three vaccines. The FDA authorized the Pfizer vaccine on December 11 for people 16 years and older and authorized the Moderna vaccine on December 18 for adults 18 and older, and most recently the J&J single dose vaccine. Both Pfizer and Moderna report that their vaccines are around 95% effective.
  • None of the vaccines are recommended over one another. However, the vaccines are generally not interchangeable with each other or with other COVID-19 vaccine products. It is important that both the first and second dose are from the same vaccine manufacturer.

Things to remember: 

  • When scheduling your first COVID vaccine dose,you are committing to receiving the second dose of vaccine at the same location. 
  • Moderna doses should be separated by 28 days; Pfizer by 21 days

PrepMod Registration

  • Select “pre-registration” sign up
  • Summit County will only be focusing on only those that have selected Summit County in their pre-registration form. 
  • Individuals that have a health condition not captured in the pre-registration form, or that do not see their occupation listed should select "other." This applies specifically to restaurant workers in Phase 1B.4.  Individuals that select "Other" in the occupation category, should also fill out this questionnaire to assist our staff with determining eligibility.
  • *Public Health is working directly with employers for government, as well as education which includes all licensed child care including home providers. Individuals in these categories should reach out to your employer.
  • Each member of your household that is in one of the currently eligible groups needs to fill out a separate pre-registration. Please use a separate email address if possible for each member of the household that is registering. 
  • Summit County is following the vaccine distribution timeline and prioritization schedule set by the Colorado Department of Public Health and Environment. All individuals age 16 and over are currently eligible.
  • Individuals do not need to complete a PrepMod registration for their second dose appointment, only for initial doses of vacine.
  • What to expect after you fill out the pre-registration
    • No further action will be required, you will be contacted by email when an appointment can be offered to you.
    • If the number of form submissions exceed the available amount of vaccine doses, appointments will be issued based on a random drawing eligible individuals. Individuals that have been on the waiting list for longer than others will not be given priority.
    • Drawings will occur as often as new doses are received, clinics scheduled and/ or cancellations occur. 
    • Those who are randomly selected to receive a vaccination will be contacted first using their email that they provided in this form to schedule an appointment for an upcoming clinic. 
    • Your name will remain on the list until it is selected in a drawing. 
    • Individuals that need technical assistance using the pre-registration form should call the Summit County COVID info line at 970-668-9730. The line is staffed from 8:00 a.m. – 5:00 p.m. Monday through Friday. Individuals will have ample opportunity to receive support with registration early next week before the lottery is drawn for the next drive-through clinic.

Registering for Appointments with Summit County Partner Providers

The PrepMod system will only be utilized for appointments at Summit County Public Health’s drive-through clinics. Eligible individuals are encouraged to reach out to partner providers in Summit County including City Market, Safeway, the Summit Community Care Clinic, and High Country Physicians group for appointments. Partner providers will be doing their own ordering of vaccine and will be receiving allocations from the State, separate from vaccine received by Summit County Public Health. 

Summit County Public Health will not longer be providing direct links to schedule for partner provider appointments, or advertising when appointments are available at partner providers. Individuals that are interested in booking an appointment with a partner provider, will need to reach out directly using the contact information below to find out availability of appointments.

Summit Community Care Clinic 

Information and registration links for SCCC’s upcoming J&J vaccination clinics:

Summit Community Care Clinic is hosting a drive-through Moderna COVID-19 vaccine clinic on Wednesday, 4/21/2021. This vaccine clinic will operate as a drive-through in the Dillon Valley Elementary School parking lot located at 108 Deer Path Rd, Dillon, CO. By registering for your 1st dose, you are committing to a 2nd dose on 5/19/21. If you cannot make an appointment on that date, we cannot guarantee your 2nd dose. Please follow the link below to complete the online registration and consent form if you are interested in receiving your 1st dose on 4/21/21.  Anyone in the community that is 18 years of age or older can register. Please show up at the exact time you schedule. Arriving even 10 minutes early will cause a large traffic queue.

If the only option available is “Waitlist”, this indicates that all appointment times are full. If you add yourself to the Waitlist, we will contact you if a dose becomes available for you on 4/21/21. If you cannot make this week’s vaccine clinic, we will have additional clinics in the future. Registration links for those future clinics will be distributed on Summit Community Care Clinic’s website and Summit County Public Health’s website.

Registro para citas con proveedores asociados del condado de Summit

El sistema PrepMod solo se utilizará para citas en las clínicas de Summit County Public Health. Se alienta a las personas elegibles a comunicarse con proveedores asociados en el condado de Summit, incluidos City Market, Safeway, Summit Community Care Clinic y High Country Physicians para citas. Summit Community Care Clinic

La clínica comunitaria del condado de Summit está organizando una clínica de vacunación Moderna COVID-19 el miércoles 4/21/2021. Esta clínica de vacunas funcionará como un servicio de autoservicio en el estacionamiento de la Dillon Valley Elementary ubicado en 108 Deer Path Rd, Dillon, CO. Al registrarse para su primera dosis, se compromete a recibir una segunda dosis el 5/19/21. Si no puede programar una cita ese día, no podemos garantizar su segunda dosis.
